Welcome to St Elizabeth Hospice and thank you for agreeing to volunteer with us.

In order to complete your application we need you to read and agree to our Volunteer Agreement and General Data Protection Regulation guidelines.

We would also like some details of who we can contact in the unlikely event of an emergency, how we can keep in touch with you, and to know whether you have any health problems which might affect you when volunteering. This form can be completed on-line by clicking the 'Apply' link below. If you would prefer a paper form please contact the volunteer team on 01473 707939.

At the end of the form there are some questions which help us measure diversity within our group of volunteers.  All questions are optional.

Promoting a Safer Hospice

We are committed to the care and protection of children, young people and adults involved in hospice care and activities. Guidance is provided to help you if you think you or someone else is at risk, the support telephone numbers are different in each county although the guidance is exactly the same.
