Our Medical Team includes Consultants in Palliative Medicine, Nurse Consultants and specialty doctors, who provide expertise in palliative and end-of-life care. They may be able to review you at home or in one of our outpatient clinics to help manage complex symptoms or advise on decision making and treatment options. They work alongside the support you get from your GP and specialists from other teams and also support the wider hospice multidisciplinary team.
Our community Clinical Nurse Specialists (CNS) have advanced knowledge and experience of palliative and end-of-life care. CNS work alongside your usual care providers, such as your GP or district nurse, to help manage your symptoms.
They support patients and families at home with symptom control, pain management, advice on difficult emotional, spiritual issues and understanding what is happening. Once the CNS has supported you with advice or treatment, and they have monitored your needs, your usual care routine will resume. This may include support from one of our healthcare professionals, your GP or district nurse or another health professional.
The Palliative Virtual Ward is a community care service which provides support to people nearing the end-of-life, who want to remain at home. People who are under the care of the Virtual Ward team receive this specialist holistic care within their own homes.
Personal care (which includes things such as washing and bathing, using the toilet, dressing and changing position) is delivered by a team of community healthcare assistants. The healthcare assistants are primarily there to provide support with this personal care. They are unable to assist with meal preparation or medication administration.
Patients under the care of the Virtual Ward team also benefit from regular oversight and support from the clinical palliative nursing team, as well as the wider community hospice multi-disciplinary team as required.
