Diversity Monitoring Form

St Elizabeth Hospice seeks to reflect diversity in terms of ethnic and cultural backgrounds, gender, age and disability. As such we ask potential employees to complete this monitoring form which will be treated confidentially and used solely to compile statistics for monitoring purposes. Upon receipt it will be separated from your application form and will not be seen by the selection panel.
