This session will explore different scenarios and stages of a person’s palliative journey and discuss when and how to begin conversations around advance care planning and end of life. At the end of the session you will be able to;
- Identify potential difficulties that may arise when initiating conversations with residents/ clients/patients and their loved ones
- Reflect on the importance of Advance Care Planning
- Illustrate why effective, compassionate communication is fundamental when having a sensitive and uncomfortable conversation with patients, their families and loved ones
- Summarise one element of learning that you will implement in practice
