Assessing the whole person

This session will equip healthcare professionals with the knowledge, skills, and confidence to undertake assessments for people with palliative and end of life care needs. Participants will explore the physical, psychological, social, spiritual, cultural, and financial dimensions of assessment and understand when and how assessment should be conducted.

The session will focus on effective communication techniques, recognising and responding to patient cues, overcoming barriers to communication, and using facilitative consultation skills to support meaningful conversations. Participants will also examine the role of advance care planning, assessment documentation, and multidisciplinary working in delivering person-centred care.

Learning Outcomes

By the end of this session, participants will be able to:

• Define holistic assessment and explain its role in palliative and end of life care.
• Identify the physical, psychological, social, spiritual, cultural, and financial needs that should be considered within a holistic assessment.
• Recognise key points in the patient journey when holistic assessment should be undertaken, including changes in condition and phase of illness.
• Demonstrate an understanding of the communication skills required to conduct effective, person-centred assessments.
• Identify common barriers to communication experienced by both healthcare professionals and patients and describe strategies to overcome them.
• Understand the principles of assessment recording, information sharing, and advance care planning within palliative and end of life care services.
• Use assessment findings to develop person-centred plans of care that reflect the individual’s goals, priorities, and support needs.

• Tuesday 13th October from 1.00pm to 4.00pm at The Education Department, St Elizabeth Hospice, 565 Foxhall Road, Ipswich, IP3 8LX

Parking

Parking is limited at all three hospice sites so we ask that you park in accordance with the highway code as poor parking creates a reputational risk for the hospices with our neighbours.  Due to the residential location of St Helena Hospice, please take note of restricted parking areas along with overflow parking option and footpath route to the hospice. Thank you for your consideration.

Cancellation Policy

Please contact us at the earliest opportunity. We ask for at least 48 hours’ notice for the cancellation of our courses. For cancellations within 48 hours of the event, any course fee paid will be retained. Fees will also be retained if you do not attend the session on the set date. All courses are subject to change or cancellation suitable notification will be provided.
