Developing Young Adult Palliative Care Services (2024)

Working with Hospice Education,  Zest Centre of Excellence at St Elizabeth Hospice, is pleased to offer this study day to share their knowledge and experience of how to develop a successful model of care to meet the needs of young adults and their families.

This session is for those working in adult hospices who are looking to develop appropriate models of care to meet the needs of young adults and their families.

The Zest Team at St Elizabeth Hospice will share their journey of developing the Zest Service, including the challenges and how they have overcome them.  The day will provide information on the Zest model of young adult palliative care and how this has been achieved including:

  • Young adult palliative care conditions and eligibility
  • Shared care model with paediatrics
  • Peer support Group programme
  • Proactive and reactive palliative care –how Zest integrate with our wider specialist palliative care adult service
  • Needs of Parents and Siblings
  • Building a capable and confident staff team
  • Short break service
  • Funding

Time will be provided for discussion and trouble-shooting with the aim of equipping and encouraging service developments that can meet the needs of young adults and their families in adult hospice settings.  As part of the day there will also be an opportunity to have a tour and visit the Zest Service.

Sessions will be delivered face to face at The Education Department, St Elizabeth Hospice, 565 Foxhall Road, Ipswich,IP3 8LX.

The cost for a session is £75.00 plus VAT.
