Would you like to be part of an exciting opportunity to join a dynamic and motivated team working with young adults living with progressive illness and complex health care needs?   Do you have a passion to make a difference to the lives of young people and their families?

Due to exciting developments within the Zest young adult care service we are looking to recruit an enthusiastic and flexible Zest Nurse/Transition Coordinator who is able to provide support to young adults 14 years upwards and their families.  This is a new role working across Suffolk and North East Essex, in the community and hospice settings.
