Salary: Band 6 (£33,706 - £40,588) pro rata

Due to exciting developments within the Zest young adult care service we are looking to recruit an enthusiastic and flexible Zest Nurse/Transition Coordinator who is able to provide support to young adults 14 years upwards and their families. This is a new role working across Suffolk and North East Essex, in the community and hospice settings. You will be able to work a combination of fixed days and flexible hours to meet the needs of the role including occasional evenings and weekends.
A key element of the role will be to enable a successful transition from children’s to adult’s services, as well as providing developmentally appropriate palliative care for young adults with complex health and care needs.
You will be part of the Zest team at St Elizabeth Hospice, working collaboratively with other services including EACH and St Helena Hospice to include developing and delivering a new monthly young adult social group in North East Essex.
