As a Care Coordinator at St Elizabeth Hospice you will help to organise care packages which will mean a quicker discharge from hospital or prevent an inappropriate admission for patients at the end of their life. Most people would not choose the hospital as their place to die, but sadly many will die in hospital. You will also support the ‘OneCall’ telephone advice line, signposting and providing information.

The hospice is working in partnership with the hospital, commissioners, and other community teams in a new initiative to give patients more choices where they receive care at the end of life.

We are looking for someone with experience of working in a busy secretarial administration role with minimum supervision.  You will need to be calm, have an empathetic manner, with excellent communication and organisational skills.

We operate a rostered shift pattern between the hours of 8am – 8pm seven days a week including bank holidays.
