Does your Care Home want to learn more about Palliative and End of Life Care?
St Elizabeth Hospice offers support to patients with palliative care needs, regardless of their location. We have now put together a package to increase the levels of support for staff and residents of Care Homes.
Our Care Homes Network is open to Care Homes with and without nursing and with 10 or more beds, located in East Suffolk.
When you sign up to participate in the Care Homes Network:
- you will commit a link person and manager to attend four study days a year and commit to the implementation of end of life care documents, training and dissemination of information and good practice;
- you will be offered a specifically designed Palliative Care Resource Pack;
- the Hospice will provide education and support to your residents and staff;
- a member of the Hospice will visit your Care Home a minimum of once a year.
The total cost for registration is £80 paid annually.
Expected outcomes:
- Improve the knowledge, confidence and skills of people working in Care Homes delivering end of life care, and therefore improve the care received by residents.
- Increased use of palliative care documents in Care Homes e.g. Liverpool Care Pathway and Advance care documents.
- More access to specialist palliative care services by residents, when needed.
- Reduction of emergency and unwanted hospital admissions of residents of Care Homes at the end of life.
- Increased number of End of life Care register patients dying in their Care Home.
Benefits to residents in Care Homes:
- The choice to stay at home at the end of life.
- To receive care from staff with an improved knowledge of palliative care as well as improved access to specialist palliative care providers.
- Improved capability to make their wishes known and a promotion of choice through Advance Care planning.
- Unwanted admissions to hospital prevented, at end of life.
