Hospice Community Nursing Teams

We aim to provide services close to people's homes or in their homes, depending on need and preference. 

Hospice at Home

Hospice at Home is a 24 hour, seven days a week reactive nurse-led service which helps to prevent unwanted admission to hospital or the Hospice when the patient and family wish to remain at home. The team provides specialist nursing care to patients in East Suffolk in their own homes alongside other community nursing services, during the final phase of their illness.

The team provides advice, support and in specific situations are able to provide specialist nursing care or symptom control as necessary.

In Great Yarmouth and Waveney the Hospice at Home service is managed by Great Yarmouth and Waveney Community Services. This service is staffed by Community Health Care Assistants.

Community Health Care Assistants

In East Suffolk Community Health Care Assistants work alongside the Nurse Specialists and the Hospice in-patient unit to provide care to patients at home. 

This service supports a discharge from the Hospice or hospital or supports patients who are living at home who have had a sudden
increase in care needs until a care package can be arranged.

This is a short term arrangement and usually statutory services will arrange carers for the patient in the longer term.

In Great Yarmouth and Waveney we provide Health Care Assistant support for up to 96 hours for patients who are living at home who have had a sudden increase in care needs until a care package can be arranged.

Community Clinical Nurse Specialists Team

Our Community Clinical Nurse Specialists work seven days a week and have advanced knowledge and experience of palliative care.

They support patients to live at home, providing a range of advice for them and their families. This includes symptom control, advice on difficult emotional and spiritual issues and understanding what is happening.

Before joining the Hospice the Clinical Nurse Specialists were limited to only seeing patients with cancer due to limited resources. Now with the Hospice’s open referral criteria they once again see patients with any progressive illness with specialist palliative care needs.

The team have recently moved their main base to St Elizabeth Hospice but still have satellite bases in Woodbridge and Stowmarket. For any queries please contact the team’s secretary on 01473 707044.
