Hospice at Home
The Hospice at Home team provides specialist nursing care to patients in their own homes alongside other community nursing services, during the final phase of their illness.

Hospice at Home is a 24 hour,seven days a week service and helps to prevent unwanted admission to hospital or hospice when the patient and family wish to remain at home. The team consists of qualified nurses with advanced knowledge and skills in hospice care and healthcare assistants. Collectively, their aim is to provide a flexible nursing service that willcomplement care being given by the Community, Macmillan and Marie Curie murses.

Who has referred me to Hospice at Home and why?
You may have been referred to the Hospice at Home team by your General Practitioner, Macmillan Nurse (Hospital or Community) or Community nurse, so that you can receive extra nursing support for yourself during the later stages of illness.
Where does my GP fit into this service?
Your local doctor will continue to prescribe your medication. The Hospice at Home team will work closely with your Community nurse and GP. Any changes to your care will be in consultation with your GP.
Does the Hospice at Home team take over from the Community nurse?
The Community nurse will continue to be your 'key' nurse and the Hospice at Home team nurses will plan with the Community nurse any extra care needed for you and your family at home.
How often will the Hospice at Home team visit?
This depends on your need, which will be assessed by a Hospice at Home nurse with your Community nurse. The Hospice at Home nurses are available 7 days a week, on call every night and in specific situations will be available to provide specialist nursing care or advice on symptom control as necessary.
How do I contact the Hospice at Home team?
Contact can be made via your: General Practitioner, Community nurse, Macmillan nurse or you can telephone 0709 2391762 to speak directly to a Hospice at Home nurse. If you have any problems getting through on this number dial the Hospice in-patient unit on 01473 707000.
